When do I go and what do I bring for new player proof of residency and/or age documentation?
New Players will not be officially registered until their age and residency are confirmed. Parents are to provide copies of the child's birth certificate and three (3) different proofs of residency, and bring it to the Jessamine Field House either on January 14, 2010 from 12pm-2pm or January 23, 2010 from 10am-12pm.
You must provide:
  • Copy of birth certificate.
  • Proof of residency documents from 3 different categories below:
            1.  Driver's License
            2.  Voter registration
            3.  School records
            4.  Wellfare/Childcare records
            5.  Federal records
            6.  State records
            7.  Local (Municipal) records
            8.  Support payment records
            9.  Homeowner or tenant records
            10. Utility bills (i.e. gas, electric, water/sewer, phone, cell phone, heating, waste disposal)
            11. Financial (loan, credit, investment, etc.) records
            12. Insurance documents
            13. Medical records
            14. Military records
            15. Internet, cable or satellite records
            16. Vehicle records
